First approved by the FDA in 2003, Cialis seemed a bit too good to be true.  A pill that treats erectile dysfunction for 36 hours, nine times the average of Viagra?  It sounded like the claims being made for “herbal Viagra” or those creepy ads with the smiling guy, snake oil products designed to cash in on the popularity of the most popular drug in the world.  So much so that sometimes suspicion of Cialis still lingers.

But make no mistake, Cialis is fully effective at treating erectile dysfunction for the time period described by the manufacturer.  Cialis is the commercial name of tadalafil, which is in the same chemical family as sidenafil (aka Viagra) and vardenafil, better known to you as Levitra, and has been the subject of multiple clinical trials.  All of these drugs work in the same way; tadalafil is just the most effective.  In fact, it’s so effective that high doses are used to treat pulmonary arterial hypertension, a condition that can cause serious damage to the heart and lungs.

At its most basic level, Cialis is a vasodilator.  Your penis becomes erect essentially through hydraulics: blood fills spongy tissues inside the penis, lengthening, stiffening and raising it.  But if the blood supply is reduced, there’s nothing to become erect with.  The spirit may be willing, but the flesh is letting you down.

Many things can reduce blood supply to the penis: arterial disease, bad diet, stress…in fact, most of the factors for heart disease also apply to erectile dysfunction, which is a way better motivator to get on that treadmill and burn some pounds than any trainer.  And Cialis won’t magically make these factors go away.  What it does do is prevent an enzyme, PDE5, from breaking down the chemical your body produces to encourage blood flow to smooth muscle tissue…such as the penis.

This in turn opens up your blood vessels, and your body takes it from there when it receives some sexual stimulation.  It’s important to note: Cialis just enables you to get an erection.  You still need stimulation in order to actually become erect.

If this sounds just like how Viagra works, well, it is: they’re essentially the same drug in terms of effect.  The difference is that tadalafil has a much longer half-life than its competitors, 17 hours compared to 4 for Viagra.  Therefore, it stays in the body much longer and keeps inhibiting PDE5 well after Viagra and Levitra have left the body.

Just like any drug, Cialis will have a different level of effectiveness for each person, and a handful of people will experience no effect at all.  There may also be problems with drug interactions that inhibit Cialis’ effectiveness, or cause other problems, so always speak with your doctor and tell him about any over-the-counter medications you may also be taking on a regular basis.

Finally, thirty-six hours or not, Cialis should never be taken with a high-fat meal, as this keeps Cialis from entering the body at normal rates: if you and your lady friend are about to have a romantic evening, have a light dinner.  Is a disappointment really worth the steak?

As a rule, Cialis starts affecting the body a half-hour to an hour after you take a dose, and the drug will stay in your system for, on average, 36 hours.  However, there are other factors that can affect how long Cialis stays in your system.

First, it’s important to remember that Cialis is a drug with a long half-life in the body, much longer than other drugs with a similar function.  The “half-life” of a drug is how long it takes for half the drug to leave the body.  Viagra has a half-life of 4 hours, while Cialis has a half-life of 17 hours.  This is why the effects of Cialis last longer than Viagra, but beyond that, there are other factors that can keep it in the body.

For example, most drugs are broken down in the kidneys and liver.  If your kidney or liver function is impaired, or you take drugs that inhibit the function of the kidneys or liver, Cialis will stay in your system longer as it is not being broken down.  Similarly, Cialis, which works as a vasodilator, may combine with other drugs you are taking which open up the blood vessels and may make the effects last longer.  Or you may pass out from low blood pressure, which is a distinct possibility.

Finally, never combine Cialis with other drugs in its class, such as Viagra (sidenafil) or Levitra (vardenafil).  These drugs may combine to cause low blood pressure or other side effects.

Beyond that, each person has a different reaction to the drug; some will see effects for a longer period of time, while others have bodies that break Cialis down faster and thus have less of an effect.

Remember, one of the most dangerous side effects of Cialis, as well as other ED drugs, is priaprism, or an erection that lasts for more than four hours.  While this may sound appealing, or like a dirty joke, it is actually a medical emergency that can require shunts or even major surgery and is extremely painful to the individual suffering it.  What happens is that if blood stays in the spongy tissues of the penis for too long, it will begin to stagnate, causing serious problems in the penis itself.  In severe cases, priaprism has forced doctors to remove some of the penis as tissues in it have begun to die and may become gangrenous.

So, remember, before taking Cialis, check each of the medications you’re already on to ensure there are no negative combinations.  This applies to both prescription and over-the-counter medications, medicinal or herbal.  Ask your doctor if there is anything you’ll need to stop taking in order to take Cialis.

Penile Suppositories:

Pellets full of medication are inserted into the urethra. Like injections, they are an unpopular alternative to Viagra / Cialis / Levitra because the suppositories may cause pain, bleeding and dizziness. The female partner may also have a reaction to the medication.

Vacuum Devices:

These mechanical devices involve a tube placed over the penis and a pump that sucks the air out of the tube. This creates a vacuum that draws blood into the penis and expands it long enough to have sex.

The technique works for most men with erection difficulties and has few side effects, but spontaneity and romance are often lost in the process.

Surgery:

Surgery for ED is usually considered as a last resort when other options have failed. The surgeon implants rods (either flexible or inflatable) that the man can manipulate to make the penis erect when needed. Mechanical breakdowns and infection are possible complications.

Surgery can also be used to open up blocked arteries and veins that deliver blood to the penis. This surgery is more useful for young men who have suffered injuries, less successful for older men with very blocked arteries.

Counseling:

Psychological therapy helps to decrease feelings of anxiety a man might associate with the sex act. This method requires patience, as it may take time, and the partner’s full cooperation and participation.

Lifestyle Changes:

This involved changing or controlling the factors that contribute to erectile dysfunction: medications that produce ED as a side effect, high blood pressure, smoking, alcohol and substance abuse, excess weight, diabetes, depression and anxiety.

Summing Up

Depending on the causes of a man’s ED and his health condition, any of the above treatments may be prescribed by his doctor or urologist.
